Umatilla County, OR. — A fatal two-vehicle crash on Highway 395A Sunday morning left one person dead and two others hospitalized with serious injuries, according to the Oregon State Police (OSP).
The crash occurred on November 2, 2025, at approximately 9:24 a.m. near milepost 3. Investigators determined that a southbound Volkswagen Passat, driven by 24-year-old Tyler John Garrett of Hermiston, crossed into the northbound lane for unknown reasons and collided head-on with a Nissan 370 driven by Hadasa Esther Sanchez, 25, of Umatilla.
Garrett was transported to a local hospital with critical injuries. Sanchez was also hospitalized with serious injuries. A passenger in the Nissan, Jose Vicente Sanchez, 24, of Umatilla, was pronounced dead at the scene.
The highway was closed for about five hours during the investigation and cleanup. The cause of the crash remains under investigation.
OSP was assisted at the scene by the Hermiston Police Department, Umatilla County Sheriff’s Office, Umatilla County Fire District 1, and the Oregon Department of Transportation (ODOT).
The Oregon State Police Collision Reconstruction Unit (CRU) responded to conduct a detailed investigation. The CRU specializes in documenting and analyzing fatal and critical injury crashes using advanced reconstruction techniques and drone-based mapping tools.
